This paper systemancally examines the strategic planning and implementanon of digital
technologies in ambulance services, focusing on the prehospital cycle from call to handover
to the ED. Three technological axes are in focus: (a) real-nme fleet tracking and management
(AVL/GIS/CAD), (b) panent monitoring and telemedicine with data integranon in ePCR, and
(c) interoperable flows to/from EDs through standards (e.g. HL7 FHIR, IHE) and secure
infrastructures (LTE/5G, edge–cloud). Business indicators are analyzed (response, on scene,
APOT, turnaround, UHU), mechanisms for reducing “lost nme” are documented (dynamic
locanon, algorithmic rounng, emergency alert, digital handover) and specificines of the
Greek context are captured (public-private sector, connecnvity, regulatory compliance). In
addinon, a comprehensive implementanon roadmap is presented: SWOT analysis, risk
management and data protecnon, measurement/improvement mechanisms (KPIs,
benchmarking, audit) and cost-benefit assessment for sustainability at scale. The role of
leadership and educanon is analyzed as a medianng factor for acceptance, standardizanon
and safe use, through change management models and the culnvanon of a safety culture.
The discussion synthesizes findings by research quesnon, highlights pracncal implicanons for
public and private actors, and suggests future direcnons (e.g., intelligent dashboards,
predicnve demand models, enhanced data governance, and ethical accountability). Overall,
it is documented that the combinanon of mature interoperability standards, resilient
connecnvity, and targeted organizanonal intervennons can translate into substannal delay
reducnon, improved quality of care, and more efficient fleet unlizanon, provided that a
coherent strategic plan and systemanc evaluanon are in place. As a contribunon, a coherent
framework of principles and stages is proposed that can be adopted by both public and
private actors with adaptability and transparency.
