Interprofessional collaboration in home health care services

Abstract
During the last twenty years many changes have taken place in the delivery of health care services especially in home care services. Specialized nursing knowledge and skills combined with modern technology offer treatment to patients with acute and chronic diseases, with safety and effectiveness at their home. A uniqueness of nursing care at home is that it is provided in an environment familial for the patient, which is controlled from the whole family. Home care is the original kind of care and to date has become a necessity. All European countries -including Greece- and countries of America have organized home care services. The main difference lies in the organization and funding of each health. Interprofessional care is a prerequisite for the effective function of every health service, because the holistic care of the individual, being healthy or not, requires almost always the involvement of more than one health professional and very often a social care one. Home care also requires the collaboration of many health professions. Home care nurses in most cases are the coordinators of all care professionals who are visiting the patient including the physiotherapist, the occupational therapist, the speech therapist, the social worker and the home care assistants. The nurse is the main point for communication and coordination of the patient care thus must use effective communication skills when in contac with other health care professionals. In conclusion, interprofessional collaboration is an approach in home health care services which will improve patient care, staff education and conducting health care research for assuring quality of the provided service.
