Abstract
The aim of this Thesis is to study the Diagnosis Related Groups system (DRGs), as integrated in the Health Systems in different countries, focusing on the efforts for the development and implementation of the system in the National Healthcare Service in Greece. The main objective of this effort was to investigate the designed steps and the methodology used for the development and implementation of the DRG system in Greece, to identify vulnerabilities and to make proposals on methods for the improvement of the efficiency and the implementation of the system in the administrative and financial management of public hospitals in Greece, based on international experience. Despite the fact that DRGs have been adopted in an increasingly large number of countries around the world, the understanding of DRG systems remains surprisingly limited. Regardless of the existence of numerous studies concerning the effects of the system on hospital efficiency, quality, transparency and technological innovation, these effects remain relatively unclear, as the specific design features in different countries are rarely taken into account. Furthermore, comparative information from different European countries as regards the specific characteristics of their DRG systems and how these characteristics contribute to achieving the above mentioned aims is largely absent. Consequently, there is no agreed consensus on how best to design DRG systems, because of the fact that the differences between countries’ systems remain poorly understood. Nevertheless, a thorough study and understanding of the international experience with DRG systems could form a useful tool used by countries when developing and optimizing their national systems.
