A study of the use of generic medicines in a hospital of Attica Region.

Aim. We sought to investigate the generic market share for volume and value in a tertiary hospital of Athens. Methods. From the 1st January 2011 to the 30th of June 2013 we evaluated the market share for total volume and value of generics in the hospital. Furthermore the generic market share for in-hospital volume and value and finally the ones for outpatient clinics were estimated. The 24 departments and clinics of the hospital were examined separately. Finally the generic market share for volume and value was calculated according to drug class as detected by ESY.net. Results. The market share for total hospital volume of generic drugs was 59% both for 2012 and the first half year of 2013. Generic market share for in-hospital volume was 55% and 56% respectively whereas for the outpatient clinic volume it was 66% in both years. The market share for total hospital value of generic drugs decreased from 36% in 2011 to 31% in 2012 and finally to 29% in the first half of 2013. Generic market share for inhospital value decreased from 41% in 2011 to 36% in 2012 and finally to 34% in the first half of 2013. Finally generics accounted for the 17% of the outpatient clinic value in 2011, 13% in 2012 and 11% in the first six months of 2013. High-costs clinics showed a 3%-18% reduction in the value of the prescribed generics between the first half of 2011 and the first half of 2013 but maintained a greater generic market share for value than the generic market share for total hospital value and the generic market share for in-hospital value. On the other hand the market share for value of generics in some mid- and low costs clinics showed an increase 3%-25%. Furthermore in clinics that incorporated very expensive therapies in every day practice the market share for value of branded medicines was found steadily over 80%. The greatest generic market share according to drug class was observed for antibiotics, 65% in 2012 and 58% in the first half of 2013 and antidepressants 49% and VIII52% respectively. Conclusion. Despite the sufficiently high generic market share for volume during the study period there was a significant reduction of generic market share for value between 2011 and the first half of 2013. However the average generic market share for value in this hospital remained close to the average one in the 2nd Regional Health Care Administration of Piraeus and Aegean Islands and in all the Regional Health Care Administrations as a whole. Expensive therapies accounted significantly for the high market share for value of licensed medicines observed in some clinics.
