The subject master thesis was chosen because of my current job position in the Hellenic Tranplant Organisation. There are about 1.100 patients in Greece with ESRD who are at the waiting list for a kidney from a cadaveric donor and the average time in the waiting list is about 7 years. Those patients cause a big financial burden to the national insurance organization because of the cost of haemodialysis treatment compared with those who have received a kidney from a cadaveric donor. The purpose of the present research is firstly to investigate all the cost factors which play major role for both renal replacement therapies (haemodialysis and kidney transplantation) and secondly to analyze separately the costs of the 1st year and of the 2nd and so on years. The time starts for haemodialysis, when the patient starts the therapy (day 1) and for transplantation, when a kidney graft from a cadaveric donor is found. Methodology: I searched for data through semi-structured interviews with representatives of all the cost factors of the 2 treatments, like nephrologists, patients, transplant surgeons and coordinators, insurance organization. Then 2 simple case studies were created in order to compare those data. Outcome: Kidney transplantation is the less costly renal replacement treatment in comparison with haemodialysis. Especially at the end of the first year, haemodialysis costs at the insurance organization 61.876 €, while kidney transplantation costs 24.590 €. From year 2 and so on, every year haemodialysis cost is 59.320 € while kidney transplantation cost is 14.315 €. Doctors’ wages are excluded.
