IMPORTANCE: Online assisted Cognitive and Behavioral Therapy (iCBT) has
the potential to reduce the cost of treating major depression with significant clinical
benefits at the same time.
OBJECTIVES: Calculation of the effect size of online assisted guided Cognitive and
Behavioral therapy in the treatment of major depressive disorder compared to alternative
control therapies
DATA SOURCES: A total of 2632 studies were identified through a systematic
literature search in databases (PubMed, Cochrane Library, PsycInfo) and grey literature
from September 15 to November 2, 2021.
INCLUSION OF STUDIES: Randomized clinical trials in which guided Cognitive and
Behavioral online therapy was compared with control therapies (CBT, care as usual,
waiting, online psychoeducation) in patients with major depressive disorder
MAIN OUTCOMES AND MEASURES: Outcomes included the Beck Depression
Scale, the Patient Health Questionnaire, and the change in depression level. Scales were
standardized for all studies included.
RESULTS: Of the 4522 participants, their mean age was 41,13 years (4.91), 68,8% of
the subjects were women, 62.2% had received education beyond compulsory while
78.4% were fully or partially employed. Online Cognitive and Behavioral therapy was
significantly more effective than the corresponding control therapies (Hedges g =- 0.49).
None of the examined independent variables had a significant impact on the outcomes.
Online Guided Cognitive and Behavioral Therapy was the most cost-effective treatment
compared to the alternative competing interventions, given the different levels of
willingness to pay of decision-makers in various countries.
CONCLUSION: Online assisted Guided Cognitive and Behavioral Therapy is clinically
effective and cost-effective in treating the symptoms of major depressive disorder.
Further policies should be adopted to ensure its diffusion into standard clinical practice.
