Abstract
In recent years, the medical record has taken on new roles. With strong concern about the cost and quality of health care, it serves as a basis for quality assurance by health care, insurance companies and other government agencies. Information technology has revolutionized chronic diseases in every aspect of our lives and has changed the standards of care by incorporating the Electronic Envelope into the daily medical practice. The electronic health record as mentioned above is the acquisition of information on a complete picture of a person's state of health. It is taken by a health care provider, a doctor or a health professional in general. An electronic medical record may contain diagnostic, laboratory and imaging examinations, medications and vaccinations, informative notes, general information about the citizen. The electronic dossier automates and simplifies the work of health scientists. With the increased incentive to document and control the provision of medical care, the use of the electronic medical record should continue to increase.
